The following requirements are mandated by the U.S. Department of State and are not negotiable:
Completed application download form (do not sign until instructed) Applicant must appear in person regardless of age
- Ages 16 to 17 with one parent named on birth certificate
- Ages 15 and under both parents or legal guardian(s) must appear in office
NOTE: If second parent or legal guardian is not available to appear in office:
- Notarized written consent of non-applying parent or guardian download Consent Form, or
- Submit original certified court order showing sole custody, or
- Submit original certified birth certificate listing only applying parent, or
- Submit original certified death certificate of non-applying parent, or
- Submit original certified adoption decree listing only the applying parent
Proof of U.S. Citizenship - Certified U.S. birth certificate
- Previous U.S. Passport for individuals age 18 and older
- Naturalization Certificate
- Born Abroad Certificate
The birth certificate must have a raised, embossed, or multi-colored seal from the city, county, or state of birth, and must show a file date reflecting that it was filed within one year of birth.
Proof of citizenship must accompany the application sent to the U.S. Department of State; however, it will be returned to you with the processed passport.
Passport Photos - 2 x 2 inch colored pictures, light colored background (no bare shoulders, uniforms or camouflage permitted in pictures). Photos available in office for $10.00.
